The 3S model offers:
The Somatic Super Syndrome (3S) is a model developed by Dr James Kustow to describe a striking pattern he observed repeatedly in his clinical practice: that many adults with ADHD also experience a cluster of physical health conditions that conventional medicine often treats in isolation, if it notices them at all.
At the core of the 3S model are three interconnected systems:
Connective tissue dysregulation: including hypermobility syndromes such as Hypermobile Ehlers-Danlos Syndrome (hEDS) and Hypermobility Spectrum Disorder (HSD). Research suggests approximately half of individuals with ADHD are hypermobile.
Dysautonomia: particularly Postural Orthostatic Tachycardia Syndrome (POTS), affecting the autonomic nervous system and contributing to fatigue, dizziness, brain fog, mood instability and disrupted sleep.
Immune hypersensitivity: including Mast Cell Activation Disease (MCAD), autoimmune tendencies, allergies, and chronic inflammatory conditions.
Together, these systems interact in ways that amplify ADHD symptoms, create additional layers of disability, and, crucially, often go entirely unrecognised. Dr Kustow’s work, presented at the European Congress of Psychiatry and featured in ADDitude Magazine, is helping change that.

Dr James Kustow BMedSci BMBS MRCPsych Dip Int Psychotherapy is a London-based Consultant Psychiatrist and the Medical Director of The Grove Practice.
He works in the Barnet NHS Adult ADHD Service, one of the few specialist NHS ADHD services in London, and holds a private practice on Harley Street. He is the Director of Education at UKAAN (UK ADHD Network), a trained integrative psychotherapist, and the winner of the ‘Genius Award’ at the 2021 Celebrating Neurodiversity Awards.
His research on the Somatic Super Syndrome – exploring the clustering of hypermobility, dysautonomia, and immune hypersensitivity in ADHD – has been presented at the European Congress of Psychiatry and featured in leading neurodiversity publications including ADDitude Magazine.
He is the author of How to Thrive with Adult ADHD: 7 Pillars for Focus, Productivity and Balance, published by Penguin in October 2024.
